On the day of moving into the Haus Mariatrost we ask you to bring along the following documents (original or copy):
Necessary documents:
- Birth certificate
- Proof of citizenship
- Marriage certificate
- Current registration form
- Current pension statement
- Confirmation of the current care level (original)
- E-Card
Additional information if available:
- current medication list.
- medical cocuments (medical reports / findings)
- name of family doctor
...and
- sufficient clothing
- toiletries
- in case of existing incontinence - bring residual stock
- in case of existing feeding tube- bring residual stock
