Who applies for outpatient rehabilitation?
Admission to the Reha Leibnitz takes place via a written application in the form of the rehabilitation application form from the social insurance institution by the insured person. For example, "outpatient psychiatric rehabilitation" should be added on the second page for the indication psychiatry and "Reha Leibnitz" should be specified.
The request is justified by the doctor (general practitioner, specialist, doctor in an inpatient facility) and forwarded to the responsible social insurance company.
If the application has been approved by the pension insurance company, we will send the insured person the date of admission and information.
The costs of rehabilitation are covered by the pension insurance institutions. The travel costs are to be borne by yourself.
Admission to our clinic takes place in the following steps:
Step 1:
Written application in the form of the rehabilitation application form from the social insurance institution by the treating doctor.
Step 2:
Please insert “outpatient psychiatric rehabilitation” in the application form on page 2 and state “Reha Leibnitz”.
Step 3:
The application is now made via the treating doctor (general practitioner, specialist, doctor in an inpatient facility).
Step 4:
After receipt of the valid permit, a suitable appointment will be made with you and information about your stay will be sent.
